What Causes Breakthrough Bleeding?

i am having breakthrough bleeding its been over a week now. went to a clinic and they did a pap and preg-test all infections and std test all negative what should i do and the doctor told me it could be stress but im not really stressed out to where i dont want to do any thing please help thank you dani
