What Causes Breathing Difficulty And Back Pain Whole On Morphine?

The VA Has had me on 45mg of morphine every 8 hours for nearly 6 years now. I have stayed on the dosage. About every 11-14 months I have an episode where I cannot breathe deep enough and require oxygen and a trip to the ER. I suffer from chronic low back pain, knee pain, and cervical pain. Lately I have had massive headaches of a Trigeminal nature causing the entire left side of my head to erupt in pain. Is the the result of being on this dosage of morphine for so long?

We have to rule out possible pulmonary causes.
