Hello and Welcome to ‘Ask A Doctor’ service. I have reviewed your query and here is my advice.

Since you are active smoker, you are at risk of heart and lung diseases. And both of them can cause breathlessness on exertion and chest pain. So better to consult doctor and get done blood pressure monitoring, ECG, 2D echo and stress test (trade mill test). All these are needed to rule out heart diseases. Also get done chest X-ray and PFT (Pulmonary Function Test). These are needed to rule out lung diseases. In any case, you should quit smoking as soon as possible. So don't wait, rush to hospital and discuss all these.

Hope I have answered your query. Let me know if I can assist you further.
