sarcoidosis.
Detailed answer: the symptoms you are describing are very typical for sarcoidosis affecting the lung. The medicines that you are taking,
prednisone,
methotrexate and advair, are "the right stuff" - standard therapy. Your doctor may be able to change your doses, or may want to add another medication, and they may want to check a chest x-ray and see you to do some breathing tests before they make any changes.

Tylenol) for the pain while you are waiting for your doctor, but I think your doctor is going to want to help you feel more able to talk and laugh, as well as concentrate.
