What Causes Cerebral Atrophy?

we have a patient with following diagnosis:

MRI imaging reveals diffuse age related cerebral atrophy with ischemic demylination/leukokoaraosis in both cerebral hemispheres and lacunar ischemic foci in bilateral gangliothalmic complexes with large confluent acute infarcts in bilateral PCA teriitories including brain stem and cerebellar hemispheres with ? normal pressure hydrocephalus
