What Causes Chest Pain And Tightness, Breathlessness, Palpitations, Back And Arm Pain?

I have tightness in my chest, very hard to breath, my heart skips beats, sometimes I have pain in my chest, back and under my left arm. I am taken a water pill for my bp which had gotten to 190 over110. Sometimes I feel as a lump is in my throat, I have had 2 bad spells like this and afterwards I feel tired, tightness in my chest, cant get enough air, tired I just want to sit down and sleep. What is this and why do I feel this way. The last 2 months I felt great until the other night when this happened again about 10:00 pm

Well come to HCM
I appreciate your concern, but here you did not mentioned your age and gender.
Overview
If you have history of hypertension and already on antihypertensive then this is matter of concern.
Workup
Cardiac evaluation is must that include EKG and stress test, ECHO X-ray of chest.
Treatment
If taking differential diagnosis into consideration then heat burn may be the likely possibility, some time stress plays the role too, and for this you need to keep the stress level low, PPI (Proton pump inhibitor ) can be tried for hyperacidity till you rule out possibility of cardiac disease.
Meditation
Tab Omeprazole 40 mg and Domperidone 30 mg once in day, Tab Chlordiazepoxide one tab three times in day.
