What Causes Chest Pain That Radiates Up To Jaw?

I get this chest pain that radiates up into my jaw. My jaw feels like a cool menthol feeling all while experiencing the chest pain. It is relieved by nitro spray. sometimes 1 spray sometimes up to 3 sprays before it takes it away. My BP goes high. I have had 3 different stress tests,angiogram, echocardiogram, EKG, blood gases and all show nothing wrong. What is it? I have have it over the past 8 years. I did oral chealation for 5 months and did not have any chest pains and or coolness in mouth or pain radiating into my jaw for several years after, now it is back.
