What Causes Chest Pain While Lying Down?

Iam 66 years old. 5'4 1/2",250 lbs. I have had two TMA and a slight heart attack after shoveling snow. I take medication for high blood pressure and take a Beta-Blocker. I take Warafin because I have had two different blood clot episodes. One went to my lungs. For the last two weeks I have chest pain while laying down. The pain will wake me up. I also cough but only in the the night. Eight weeks ago I had surgery on my knee for two torn meniscus. That went well.

I can understand your concern.
In my opinion, we should definitely rule out recurrence of pulmonary embolism and coronary artery disease (CAD) for your chest pain.
You had history of pulmonary embolism (PE) and recently had surgery. So chances of recurrence of PE are more.
You are also having high blood pressure and mild heart attack previously. So coronary artery CAD at present can also cause similar symptoms.
So better to consult doctor and get done blood pressure monitoring, Ecg, 2d echo, stress test and CT pulmonary angiography to rule out these possibilities.
