What Causes Chest Pain With Pressure?

I have had center chest pain like someone is sitting on my chest for the last 3 weeks on and off. Pain that goes down to my right elbow and upper right of my back. Get winded real easy where I didn't before. Sometimes the chest pain goes away with relaxation but sometimes it's worse laying down.
