What Causes Choking Up On Food And Pain Below The Upper Clavicle?

Hello, 23 year old female here. I am a smoker, about a pack a day for the past 4 years. About a month ago I started having complications with eating. I will very commonly choke on my food and have to force food to dislodge from my throat by gagging. When I gag, water will pour out of my throat and push the food out little by little. Also, I cough after every meal. Then about three days ago, I started feeling a very sharp pain right below my upper left clavicle every time I breathe in. It will occasionally go away for a few minutes at a time, then it comes back. I am unsure of whether these symptoms are all related or if I should be concerned enough to see a specialist.
