What Causes Chronic Shoulder Pain?

I have chronic shoulder pain - however it comes and goes. I sometimes sleep with my arm over my head that seems to make it worse. A couple of times when i would wake up in pain after getting up the pain would suddenly go away. Any idea what this could be?
