What Causes Constant Arm Pain With History Of Fractured Elbow?

I suffered a Fractured (elbow) Humerus 20 years ago at 28 years old. Now getting constant aching and seemingly associated arm pain. Pain moves towards chest if pressure applied with hand... I.E car gear stick.
5ft 11 ins. 15 stone. None smoker.
