What Causes Constant Chest Pain And Shortness Of Breath Upon Walking?

I am 28 year old boy had an habit of smoking from 2007 to 2011. Now having constant pain in the middle of the chest from last 2 months and shorting of breath even if I walk fast for about say 100 m. Thorax report is normal on 15 Aug 2014. Chest pain continues even after normal report of thorax and getting treatment from the doctor.

Thanks for your question on HCM. In my opinion you should first rule out cardiac and respiratory cause as you smoked for almost 5 years.
So please get done
1. ECG and 2D Echo to rule out cardiac causes.
2. Chest x ray and PFT (Pulmonary FunctionTest) to rule out pulmonary causes like chronic bronchitis.
If all of the above are normal than it must be anxiety and stress related.
Stress and anxiety can cause shirtness of breath and middle chest pain.
