Hi, From history it seems that there might be having some degenerative changes in your lower vertebrae producing pinched nerve pressure giving rise sciatica pain. Go for x-ray lumbo-sacral region for spondilitis. After report physiotherapy, short way diathermy as advised by orthopedic surgeon. Take calcium and vitamin D supplements. Ok and take care.
