What Causes Constant Throat Clearing, Acid Reflux, Hernia And Stomach Gas?

I have been suffering from chronic throat-clearing for the past 12 years. Feels like I m choking most of the time. Keeps me up through the night also, so I am exhausted all the time. I have been to an ENT doctor, and was prescribed Nexium, after 3 months, still no relief, so I stopped the pills. I have acid reflux and a hiatal hernia. I also have a lot of stomach gas. I have to sit up at night to relieve the stomach gas. My throat clearing also triggers asthma. Help!
