What Causes Continuous Heavy Bleeding With Clotting?

i have been bleeding for 5 days now and bleeding is heavier during morning with blood clots. i am taking a pill named exacyl tranexamic acid for 2 days to stop the bleeding but it just dont work i still bleed heavily. what should i do please help by the way im taking the pill 3 times a day.
