What Causes Cracks On Top Joint Of Shaft Of Penis?

right on the top joint of shaft to head bottom of penis showing some cracks and with white dry discharge accumulated. No inflamation but mild pain while it get enlarged or if I have sex. Usually it takes 5 - 6 days to get healed and it happens in this seasons only. Please guide

It can be due to phimosis and imbalance in blood glucose level. Rule out for diabetes.
