What Causes Delay In Periods But Light Spotting In Between?

Hello,
My sister has not had a peirod in two months. About five weeks ago she started spotting. Then tonight she went to the restroom and felt like she had to push and some came out that was about quarter size and was about seven inches long did she have a miscarriage?
