What Causes Discomfort In Right Shoulder, Elbow, Neck And Head?

I have a very strong discomfort in my right shoulder. The discomfort goes from the middle of my elbow, shoots up to my shoulder into the right side of my neck and into my head. the discomfort is undescribable. I only feel this discomfort when i get sleepy.
