What Causes Dizziness Followed By Chest Pain?

OK, so Every few days- a week I get suddenly dizziness and sometimes blackout. This is followed by sever chest pain, palpitations, pain/ numbness in left arm,blue lips and hands,extreme fatigue and a weird metallic taste. This has been going on for a few years and I have received medical care for Costochonditis (??) and Tietze syndrome but did some research and neither explain the other symptoms, some doctor said It was because the pain makes me hyperventelate but i, since then, have controlled my breathing and it still occurs..
I'm 17, female, student with no other health issues but as often ill ( viral menigitus, fevers, glangular fever ect).. whats up?
