What Causes Dizziness, High Blood Pressure And Lightheadedness?

well ive been feeling kinda lightheaded and some what dizzy my blood pressure is ok its 125 over like 84,but some times i feel like that when i play ball or hen im just walking but if i play ball for a while i can feel it coming my lightheadedness.so i have to stop and rest i work out and i try to stay in shape.well one day i got sick of it so went to the doctor and they looked inside my ears and they said i had clooged ears on both sides so they cleaned them out and go figure it went away for like three days no pressure in my head or nothing i could play basketball for a long time without stoping,then on the third day i was playing and it hit me hard i just felt bad and lightheaded and things looked tilted on the court i said no!!my head got pressure,then its been with me for a while again i tryed to clean out my ears myself but no effect what so ever,i think it my sinuses cause i feel pressure in my eyes and head and top of my head and sometimes i get headachs,so ive been feeling like this for like 5 or 6 years now so if you can tell me what u think is wrong i would appreciated thanks!!
