What Causes Elevated Esr Rate With Normal WBC In HIV Patient?

hiv patient with elevated esr rate (146) and crp but a normal wbc. Recently changed HIV meds because of drop in CD4 count rom 380's to 180's. Kidney, heart, and other systems are all normal. BP 110/72. Denies fever. Bilateral lower leg swelling that is painful. Patient feels tingling and numbess. Purplish/Red discoloration. No history of immune disease, no obvious signs of infection, what is causing this?

In hiv pt. if pt is on zidovudine based regimen then there is chance of anaemia and esr can be heigher. But along with this crp is is also raised and CD4 COUNT decreased so it point towards some opportunistic infection. As raised esr can be seen in tuberculosis so better you go for screening for pulmonary and extrapulmonary tuberculosis along with other opportunistic infections.
It is always advisable to rule out OI IF CD4 are decresing before changing over to next regimen untill and unless it is surely a immunological or virological failure.
