What Causes Elevated Heart Rate Despite Taking Lisinopril?

my blood pressure is 223/104 pulse is 44 to 101. I take lisinopril 20 mg daily and my dr has changed it o 20 mg bid and there is only minimal changes in bp. lab result are wnl what is going on with me I am 73 years old 5 3 and I weigh 230 lbs with sudden weight gain of 10 lbs

Hi thanks for asking question.

You have severe degree of hypertension.

We have to rule out following possibility.
-kidney problem induced hypertension
-endocrine disorder
- neurological cause or
- life style related hypertension.

Sometime if kidney condition like glomerulopathy or glomerulonephritis associated then hypertension can be severe.Urine show proteinuria.USG or biopsy helpful if suspected.

Sedentary life style , lack of activity and lack of exercise can lead hypertension to severe degree.

Rule out history regarding endocrine disorder like loss of libido, gynecomastia, infertility, striea, facial change etc.

Avoid stress.

Continue drug.

If symptoms of organ damage seen or it seems to be risk for organ damage with papilloedema in eye , intravenous nitroprusside can given.

Better to take 2 drug combination rather then taking lisinopril alone.
Diuretic or beta blocker or calcium channel blocker like drug can be added.

If dizziness comes with slurred speech rule out stroke by CT scan.
