What Causes Enlarged Lymph Nodes In Neck?

I recently had a ct done and it shows enlarged lymph nodes in the neck. Largest 1.3 x 0.8 cm. The thyroid is full of multiple nodules. The neck has been like this for about 4 years, and slowly they're getting bigger. Carotid artery blockage of 70% on both. Plus, my hearing in the left ear is way down. Throughout this time I went through numerous sets of ear tubes, because the fluid just pours out of my left ear. Without the tube it builds up in the mastoid area behind the ear, then lastly here it started coming out the left eye. Each morning my eye would be swollen and glued shut. I have HBP, yet I'm only 45. Been this way for twenty years. I'm 5'7 and 110lbs. I like to think fairly active. The headaches have changed the past while. They seem to come from the center of my head deep inside and the lower back part of the skull. My ENT dismissed all this at our appointment yesterday. He just said the sinus cavities are clear. Should I try to get a consult with another ENT or no.
