What Causes Excessive Bleeding And Clotting?

I have been bleeding for 4 and 1/2 weeks, on bc for now 3 months. I have been severely anemic in the past and now I'm dizzy and weak and am changing a pad every hour to hour and half! The blood is bright red and I've pasted tissue like half dollar size clots and I know I'm not having an misscarriage
