What Causes Excruciating Pain Having DDD At L5-S1?

MRI states DDD at L5-S1 withcentral disk extrusion with left sided enfacement of the nerve root. I have had 3 epidural injections in the last 4 months that has not helped. Pain is excruciating. What does this mean? Is it serious? Would a discoptomy correct it?
