What Causes Extreme Pain In Bladder Radiating To Testicles?

Hi, the symptoms have been happening since I ve been about 24, I am 37 now. I get extreme pain in my bladder reach in and below my testicles. It hurts so bad that I have to get on my hands and knees and not move. Sometimes it lasts only 10 or 20 minutes and other times it lasts up to 45 to an hour. The first time was after I went on a 45 mile bike ride and didn t drink very much water. It has happened after I have been sick for multiple days and using the bathroom frequently (diarrhea and every ten to 20 minutes). Other times it happens when I ejaculate, perhaps too quickly and stop it before it all comes out. Please help Doctor(s). YYYY@YYYY Thank you in advance, Jack

Hello,

You are suffering from extreme pain in the bladder radiating to the testicles. You also experience pain when you ejaculate. Such symptoms are caused due to infection, inflammation and pain in the prostate gland. Other medical conditions that can cause it are UTI, pelvic floor muscle spasms, bladder infection, bladder stones, prostrate stones, stress, testicular torsion, hernia, etc.

You are suggested to get clinically examined by a Urologist, get Urinalysis(R/E & C/S) to check for UTIs. Blood tests done to measures PSA. High levels may indicate prostatitis, BPH. Your treating doctor may also tell you to get done cystoscopy, ultrasound, transrectal ultrasound, etc., to come to a diagnosis.

If UTI is detected you will be treated with antibiotics to control the spread of infection. Chronic prostatitis may require conservative treatment for 2-3 months. Acute bacterial prostatitis may need 14 to 30 days of antibiotics and analgesics. You can take Naproxen or Combiflam, one stat when needed to reduce pain.

If stones are detected the treating doctor will suggest surgery to remove prostate stones or scar tissue in the urethra. Stay well hydrated and avoid stress as it may cause neurologic pain. You can take amitriptyline and gabapentin for relieve from neurogenic pain. Get treated at the earliest.
