What Causes Extreme Pain In The Elbow Radiating Towards The Forearm And Hand?

extreme pain in elbow all the time, especially when I bend it or straighten it out. I have pain that travels to middle upper outside of arm and to middle of inside forearm. I also am experience some swelling in hand and fingers. I have been icing and using heat,not working.
