What Causes Extremely Dark Urine While Suffering From IGA Nephropathy?

Hello my name is brad I m 20 years old and male. I was told to email you on the kidney website, well the other year I went proper ill for about a week headaches cold sweating dizzy throat infection dark urine, this happened twice that year. The doctor did a urine and blood test and it came back that my kidneys were leaking blood and protein into my urine, after that they decided to do a kidney biopsy and it came back I had iga nephropathy. The kidney specialist said the scarring wasn t that bad now I go back every 6 months for checkup and take 2.5mg ramipril daily and not much else was said about it. But after looking on the internet I don t know wether there s things I should or shouldn t be doing? I mean they said the scarring wasn t that bad so does that mean I don t have to change my diet? But now this week I m going through that I ll episode I had the other year and my urine has gone very dark red again I went to the my local doctor today but he seemed comfused about it and told me to come in tomorrow for blood and urine test and give me antibiotics as turns out I have tonsillitis aswell, but I don t see how that s that s helping the problem now? I mean isn t this making my kidneys worse. I d be very thankful for any advice

Hi,

Dark urine in a diagnosed case of IgA nephropathy indicates gross hematurea (blood leaking in urine). This can happen episodically, especially following minor febrile illnesses like upper respiratory infection or gastroenteritis.

This condition is treated with drugs reducing protein leakage in urine (like ramipril you are already taking), and closely monitoring and treating hypertension if any. When there is progressive increase in protein leakage or worsening renal function despite this, we need to go for other drugs like corticosteroids or immuno modulatory drugs.

Tonsillitis (usually associated with fever / throat pain), if diagnosed, you can complete the antibiotic course- penicillin antibiotics usually will not cause kidney damage. But analgesics other than paracetamol are better avoided. Also restrict salt intake, processed food items, and include omega 3 fatty acids (like fish oil) in diet, as it is known to slow down progression of disease.
