What Causes Fear Of Loud Sounds And Delayed Speech In A Child?

my 2 yr old great grandson does not speak, and is terrified of noises, ie a hairdryer, leaf bower, lawnmower. He often does not seem to be aware of his surroundings and walks into things (furniture, door jams). I am concerned, but his parents say they have noticed nothing. Am I just being a nervous Nellie? Thank you.
