What Causes Fever With Chills?

Every afternoon around 4,5 o clock i get chills and temp. after 99.6??? This has been going on for a few weeks now. Been to the E.R., Did blood tests, x-rays took Cipro for 5 days, but the problem continues,????? What could cause it? I am 85 yrs. olds.
