What Causes Fluctuating Blood Pressure Levels?

I have been having trouble with my blood pressure. I have been doctoring and know one seems to know what the underlaying problem is. a couple of weeks a ago I had a tia and a heat attack. . but this problem with the bllod pressure has been going on now for a year. it cAN BE LOW AS IN 72/56 AND THEN IN A COUPLE HRS LATER IT CAN BE HIGH AS IN 189/112AND HIGHER then I can have a awesome normal as in a 120/70. I am on high blood pressure meds. and I have been on them for years. can you help me figure out what is going on?
