What Causes Frequent Hiccups In An Elderly Person With Slow Kidney Function?

My 91 yr. old father has had frequent spells of strong hiccupping over the last few months. Is this a symptom of something going on? Also, his doctor prescribed Zoloft a couple of weeks ago because he was despondent. He went to his doctor in Jan. because of a general feeling of just not feeling well and all blood work and EKGs came out well with the exception that his kidneys were working slow because of his dislike for water. He has an appointment this Monday with his doctor again. Any thoughts?

Dear
Thanks for contacting HCM with your health concern and following Ia stepwise symtpomwise managment:

1. Hiccups in an old male (91 years) can be due to to things:

i. either raised blood urea due to reduced kidney function and/or
ii. due to prostatic enlargement causing urinary obstruction and raising blood urea, so ask for prostatic symptoms also.

PS. Urea irritates the phrenic nerve and thereby causes hiccups.

2. Since Hiccups are persistent and recurrent, ask for
. Blood Urea & S. Creatinine
. X-ray Chest, X-ray KUB
. Gastroscopy
. Ultrasonography of Liver.

3. Till then:
. Mucaine gel 2 tablespoon 2-4 hourly ( Antacid + Anaesthetic)
. Tablet Baralgan 1 thrice daily (antispasmodic)
. Tablet Alprazolam 0.25 mg thrice daily (if anxiety)
. At home: breath into plastic bag- held over nose & mouth
. Hold breath as long as possible
. Hold breath, pinch the nose & drink water
. Give ice cold water or ice cream to eat

4..If Hiccups still not controlled

. Inj. Baralgan (Drotaverine) 3 cc I.M. or 5 cc I.V.
. Xylocain viscous 30 ml to drink. May be repeated after 4 hours.
. Stomach wash with Ice- cold saline + 1% soda bicarbonate
