What Causes Gradual Reduction In Weight?

a pt 30yr c/o secondary amenorea -7yrs, pain lower abdomen on and off, allergy to sun light - 2yrs. 2 live issue before sec amenorea. past mentrual history normal. pt had vaginitis in past.pt had MTP & tubectomy 7.5yrs back. after that pt had menstruation for 2-3 times in 6month and then spoting sometime and finaly gone. USG done normal. FSH , LH,TSH & estrogen normal. TBPCR with withdrawl bleed 2yr back shows NTMB.But now pt is not having withdrawl bleed with estrogen and progesteron.pt lose her wt since 7 yrs about 3 kg. HIV neg.now wt is 45 kg.pls guide management.
