What Causes Gradual Rise In A1C Values?

My A1C is 11.9 from 6.2 three months ago. I have not done anything much different - I take januvia, glum buried and metformin daily. My reading from my meter is between 105 and 135 in the morning before breakfast , and 158 to 179 after dinner befor I go to bed. My doctor is suggesting insulin but wants to see another reading in three months. What is the reasons for the A1C spike over these three months?
