What Causes Grinding Feeling In Neck While Turning?

I have been to a massage therapist and the chiropractor but I still have this feeling in my neck every time I turn my head or lean it to one side or the other. It feels like it's grinding and I hear a crackling sound. I also have pain in the shoulders and back that ulternates from one shoulder blade to the other. Any ideas?
