What Causes Grinding Of Shoulders While Arm Movement?

I'm 15 years old, and for years now every time I move my arms, my shoulders grind. It feels uncomfortable, and after it hurts. I used to play a lot of sports like hockey, and softball, and I was a pitcher is softball but I had to stop pitching because it became too painful. Do you have any idea what this could be?
