What Causes Hard Lump During Ear Infection In An Infant?

My 8 month old has a hard lump but slightly moveable behind her ear. She has an ear infection and has been on antibiotics for 5 days. I notice it once before when she was 3 months and her dr told me it was fine and a lymph node which can show up during illness or infection could that be what is happening now? Should I be worried?
