What Causes Head And Shoulder Pains?

I am not very sure with what is happening with me, but its many times that I have observed that whenever I have a neck and shoulder pain, there starts headache (on the Back of my Head) and I feel that my blood pressure is increasing. I feel very weak and sleepy. What is it?
