Hi welcome to health care magic.....

Here most noticeable factor seems to lead high blood pressure is dear your smoking habit.....

Smoking can lead endothelial damage , vasoconstriction, free radical formation all lead predisposition for hypertension and thrombosis....

Smoking in addition release elastase which damage elastic tissue of lung and can lead chronic obstructive lung disease or fibrosing alveolitis like restrictive diadease..
Chest x ray and spirometry done for it...

So gradually decrease frequency and stop smoking.
Antihypertensive medicine taken regularly...

Avoid stress....

Continue daily exercise.
More fruits and green leafy veg useful.

Also investigate with ECG if chest pain associated with radiation to arm or neck.

Take care.
