What Causes Heavy Continuous Bleeding With Menstruation?

I am suffering from heavy bleeding from 14-Nov. Last month also same thing happened I was bleeding around 15 days from 5th Oct - 19th Oct. After a month of May I got my period on 5th Oct and this month on 14 -Nov but its still bleeding. I have stop taking below tablet before 4 days. I was taking Hyponidd (Twice in a day from month of May 2010) and Premence plus(one in a day from May 2010).
