What Causes Heavy Vaginal Bleeding And Bloody Urination?

I am a 67 year old white female. I have been experiencing the passing of large blood clots for several months now. It is random, and usually lasts about 2-3 days. This time, however, I am on day 8. I seem to have lost bladder control and have to urinate about every 20 to 30 minutes. I also have a bad yeast infection that is very painful. I have tried otc treatment but it has not helped. I am up to using 20 pads a day and there is always blood in my urine. My urine also has a bloody tissuey substance in it.
