What Causes Heavy Vaginal Bleeding While On Oral Contraceptive Pills?

After starting the OCP and taking 3 months in a row without a break i have now a break through bleed and this morning passed 2 pieces of tissue looking things that werent blood clots - is this endometrial wall passing thickened as i havnt had a period for 3 months?
