What Causes Heavy Vaginal Bleeding With Clots?

i had a baby july 5 2010 tonight i went to the bathroom and i passed a blood clot now my blood is coming out like water im changeing my pad every 5 mintues. and now again i went to the bathroom and i passed 2 more blood clots what does that mean? or what should i do cause my period is still coming out like water
