What Causes Hiatus Hernia?

I am female aged 82. I have Marfans syndrome, I have scoliosis, I have a hiatus hernia. I have a pain in the back a little below the shoulder blades at the spine, maybe a little to the right. The pain radiates. The impression is that something is pressing the spine from the inside. I have been belching a lot recently.
