General & Family Physician 's  Response
Hi & Welcome.
I appreciate your concern for your beloved wife.

A high ESR associated with painful joints is suggestive of acute flare up of the inflammatory arthritis like rheumatoid arthritis.

She needs to be evaluated by a rheumatologist for certain lab tests such as Rh factor, C reactive protein, ASO titre, Anti-nuclear antibodies, Anti-smooth muscle antibodies, Anti-CCP, etc.

She may need a short course of Prednisone followed by anti rheumatoid drugs like Methotrexate or Hydrooxychloroquine for her painful joints.

Hope this will help.
