What Causes High Fever Along With Breathing Difficulty?

My brother presented to the emergency room at the hospital weak out one week lqter of breath and with fever of 103 brother is 77! He destroyed two emergency rooms in two different hospitals was out of his head fo four days! Now the doctors have revived him from the mania but his oxygen levels refuse to come back to normal
