Hello and Welcome to ‘Ask A Doctor’ service. I have reviewed your query and here are my thoughts.
There is a history of persistent swelling in the right temporal area which increases and also gives pain on exercising indicate one of the reasons for possible symptoms. Vascular that is arterio-venous fistula; this increases on exercises as the blood supply is increased to this area causing it to enlarge and cause pain also. Hence I would advise you the following in such a situation: Get an appointment of a Surgeon for clinical examination. Ultrasonography with color doppler can give fairly correct diagnosis as to why is the problem. Rarely MRIAngiography may be needed to get a confirmed diagnosis so that a proper management can be planned, surgery if needed.
Hope I have answered your query. Let me know if I can assist you further.
