What Causes Indentation On Trapezius Muscle With Pain And Cramps?

I have an indentation in a section of my trapezius muscle on the left side. This area has had pain and spasm for past year and a half. I had botox shots to relieve the spasm in 1/2001. I injured my left shoulder ( tendinitis and sprain) in early 12/2010. What could be causing the indentation?
