What Causes Intermenstrual Bleeding?

I seem to be experiencing what I consider to be a double period. I have my usual period, and when it seems like it is going to finish (after 4 days), I get a heavy period with a lot of clotted blood discharged. I have been menstuating for 18 years so I know it's not normal for me. Do you have any idea what it could be?
